Position Title:            Housing Case Manager         

Reports To:                Connect 2 Home Manager                            

Broad Function:        Provide outreach, case management, and supportive advocacy for families                                        within the Connect 2 Home

III.  SUPERVISION:
  1. Attends supervision consistently and comes prepared.
  2. Documents supervision.
  3. Able to receive feedback and to learn from it.
  4. Has insight about self (i.e. strengths and weaknesses) and utilizes that knowledge toward growth and change.
IV.  HOUSING CASE MANAGEMENT:
  1. Provide outreach and case management to youth and provide input of case management as part of multi-disciplinary team.
  2. Works with youth to identify housing goals and preferences.
  3. Coordinates the intake process and identifies housing for youth referred to C2H program.
  4. Provide case management specific to housing and the needs of youth/youth’s families referred to C2H program
  5. Maintain communication with property managers to uphold the stability of C2H youth.
  6. Provide appropriate follow-up to best support the youth’s needs. Re-assess strengths and needs, in partnership with the family, as they meet goals and establish new goals over time.
  7. Ensure that youth enrolled in C2H program are educated on rights and responsibilities of tenancy.  
  8. Maintain required documentation of services provided and coordinated for youth.
  9. Attend all meetings, trainings, and workshops as required by program.
  10. Possess knowledge of community resources and public assistance programs and the ability to assess, refer, and link clients to those services.
  11. Develop and manage a comprehensive outreach/ working relationship with stakeholders including other community members, especially property management and landlords.
  12. Implement a property management/landlord recruitment and retention plan. 
  13.  Maintain awareness of changes in market and community processes that can impact services; bring forward prospective recommendations to deal with expected changes.
  14. Demonstrates constructive advocacy techniques and support for youth on an as needed basis.
  15. Delivers ongoing housing stability support for youth referred to C2H program.
  16. Provide financial literacy education to youth to support housing stability. 
  17. Complete housing inspections for youth enrolled once a month.
IV.  TRANSPORTATION:
  1. Maintain a professional and safe environment in vehicles. Ensure the safety of children and adults by providing appropriate safety measures such as seat belts, car seats, and child safety locks.
  2. Responsible for keeping the Transportation Signature Log and notifying Manager, in a timely fashion, of any problems.
  3. Responsible for monitoring of vehicle maintenance and documentation of such maintenance, including regular service appointments and weekly cleaning.  Inform manager or identified liaison of any problems with the vehicle in a timely manner.
  4. Responsible for maintaining Mileage logs and for keeping records of any receipts.

Specifications:
Must have valid New Jersey state driver’s license.

Education: Associate’s or Bachelor’s degree in the Human Service Field or Social Work.

Experience:  One-to-two years working with client population providing case management. Knowledge of

Knowledge:  Knowledge of community resources, assessments, care plans, progress notes, and computer database systems.

Skills and Abilities:  Ability to assess client’s needs; provide assistance in obtaining concrete services and the ability to complete all paperwork in a timely manner. Good written and oral communication skills. Ability to create a warm, caring, non-threatening therapeutic atmosphere. The willingness to work with individuals from diverse backgrounds. An understanding of professional ethics and issues of confidentiality. The ability to work on a multi-disciplinary team. Possess the ability to use a computer database system.

Work Environment/Physical Demands: The individual must possess the ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, and lifting of up to or in excess of 50 pounds.
